Công cụ khảo sát thực trạng bao gồm ba loại phiếu: Phiếu hỏi ý kiến số 1, Phiếu phỏng vấn (phần 1 và 2), Phiếu quan sát.
* Nội dung phiếu hỏi ý kiến số 1 (Phụ lục 1) bao gồm:
- Phần 1: Thực trạng HOẠT ĐỘNG GIÁO DỤC KỸ NĂNG SỐNG cho HỌC SINH.
Câu 1: Đánh giá chung về trình độ KỸ NĂNG SỐNG của HỌC SINH và 5 KỸ NĂNG SỐNG cụ thể theo 5 mức: tốt, khá, trung bình, yếu, kém. Câu 2, 3, 4 và 5: Khảo sát mức độ thường xuyên và hiệu quả của việc thực hiện các nội dung, hình thức, phương pháp, phối hợp các lực lượng giáo dục trong HOẠT ĐỘNG GIÁO DỤC KỸ NĂNG SỐNG theo 4 mức: không làm, thỉnh thoảng, thường xuyên, rất thường xuyên và theo 4 mức: không hiệu quả, ít hiệu quả, hiệu quả, rất hiệu quả.
- Phần 2: Thực trạng quản lý HOẠT ĐỘNG GIÁO DỤC KỸ NĂNG SỐNG cho HỌC SINH.
Câu 6: Khảo sát mức độ thường xuyên và hiệu quả của công tác quản lý HOẠT ĐỘNG GIÁO DỤC KỸ NĂNG SỐNG gồm 4 nhóm công việc ứng với 4 chức năng quản lý với 34 công việc cụ thể, đánh giá theo 4 mức: không làm, thỉnh thoảng, thường xuyên, rất thường xuyên và theo 4 mức: không hiệu quả, ít hiệu quả, hiệu quả, rất hiệu quả. Câu 7: Khảo sát nguyên nhân gây nên hạn chế của HOẠT ĐỘNG GIÁO DỤC KỸ NĂNG SỐNG và quản lý HOẠT ĐỘNG GIÁO DỤC KỸ NĂNG SỐNG, theo 5 mức: hoàn toàn không đồng ý, không đồng ý, lưỡng lự, đồng ý, hoàn toàn đồng ý.
