1. Khi đi khám, vui lòng mang theo

1. Khi đi khám, vui lòng mang theo 2hình 4x6, nền màu trắng, không đeo kiếng và passport.
2. Chi phí khám là 1.210.000 VNĐ / người.
3. Thời gian khám là 1-2 tiếng đồng hồ.
4. Kết quả sẽ được trả sau 2 ngày khám (trừ ngày chủ nhật và ngày lễ), nếu có xuất hoá đơn VAT sẽ được trả kèm cùng kết quả. Vui lòng đến bệnh viện lấy kết quả.
5. Dịch vụ chuyển phát Hồ Sơ EMS: 20.000VNĐ đối với nội thành, 30.000VNĐ đối với ngoại thành. Vui lòng chờ 4 – 5 ngày làm việc để nhận được Hồ Sơ kể từ ngày khám.
6. Địa chỉ bệnh viện Đa khoa Vạn Hạnh: 781/B1-B3-B5 Lê Hồng Phong (nd) - P.12 - Q.10, lên lầu 4 Khu B - Phòng khám theo hẹn.
7. Nhịn ăn trước 4 tiếng để lấy máu làm xét nghiệm.
8. Thời gian làm việc: Sáng: 7:00AM – 11:30AM, Chiều: 01:30PM – 5:00PM (Chủ nhật và ngày lễ nghỉ).
