Chúng tôi là: Chi nhánh Công ty TNHH MTV - Tổng công ty phát điện 3 - Công ty Nhiệt điện Mông Dương (Viết tắt là: Công ty Nhiệt điện Mông Dương) - Thuộc Tập đoàn Điện lực Việt nam, là Chủ đầu tư Dự án “Nhà máy nhiệt điện Mông Dương 1, công suất 1000MW” (sau đây gọi tắt là Dự án) đã được phê duyệt báo cáo ĐTM theo Quyết định số 136/QĐ-BTNMT ngày 2 tháng 2 năm 2007 của Bộ Tài nguyên và Môi trường.

Tuân thủ theo Luật Bảo vệ môi trường và Quyết định số 136/QĐ-BTNMT ngày 2 tháng 2 năm 2007 của Bộ Tài nguyên và Môi trường. Chúng tôi xin gửi đến quý Bộ Tài nguyên và Môi trường hồ sơ đề nghị xác nhận hoàn thành các hạng mục công trình, biện pháp bảo vệ môi trường phục vụ giai đoạn vận hành dự án, bao gồm:
Một (01) Bản sao Quyết định số 136/QĐ-BTNMT ngày 2 tháng 2 năm 2007 của Bộ Tài nguyên và Môi trường. Về việc phê duyệt báo cáo ĐTM của dự án “Nhà máy nhiệt điện Mông Dương 1, công suất 1000MW”. Kèm theo bản sao báo cáo ĐTM của Dự án đã được Bộ Tài nguyên và Môi trường phê duyệt.
– Năm (05) bản báo cáo kết quả thực hiện các công trình, biện pháp bảo vệ môi trường phục vụ giai đoạn vận hành Dự án theo quy định.
– Năm (05) bộ phụ lục kèm theo báo cáo kết quả thực hiện các công trình, biện pháp bảo vệ môi trường phục vụ giai đoạn vận hành của dự án, bao gồm:
+ Bản sao giấy chứng nhận đầu tư số 22.121.000.277 do Bộ Kế hoạch và Đầu tư cấp ngày 15/3/2012.
+ Bản sao giấy chứng nhận hoạt động chi nhánh 3502208399-008, do Sở kế hoạch và đầu tư tỉnh Quảng Ninh cấp cho Công ty điện lực Mông Dương, ngày 16 tháng 3 năm 2015.
+ Bản sao giấy phép khai thác và sử dụng nước biển số 2004/GP-BTNMT, do Bộ Tài nguyên và Môi trường cấp ngày 10 tháng 8 năm 2015.
+ Bản sao Giấy phép xả nước thải vào nguồn nước số /GP-BTNMT , ngày tháng năm 2015, do Bộ Tài nguyên và Môi trường cấp. Kèm theo bản sao Đề án xả nước thải vào nguồn nước của “Nhà máy nhiệt điện Mông Dương 1, 1000MW” 2 ”.
+ Bản sao sổ đăng ký chủ nguồn thải chất thải nguy hại số …………. do Sở Tài nguyên và Môi trường tỉnh Quảng Ninh cấp ngày … tháng …năm 20…. (Kèm theo các chứng từ chất thải nguy hại kèm theo đến tháng 11 năm 2015 ).

+ Biên bản bàn giao, nghiệm thu công trình xây dựng Nhà máy nhiệt điện Mông Dương 1 giữa Công ty nhiệt điện Mông Dương và Công ty TNHH HUYNDAI E&C. Kèm theo toàn bộ hồ sơ hoàn công các hạng mục công trình của dự án .
+ Các phiếu lấy mẫu và kết quả đo đạc, phân tích mẫu vận hành thử nghiệm các công trình xử lý chất thải của Dự án do Công ty cổ phần Kỹ thuật và Công trình môi trường phối hợp cùng Trạm quan trắc môi trường Phía Bắc thực hiện tháng 9 - 10/2015.
