3. Thực hiện định nhóm máu hệ ABO của người bệnh, của túi máu ngay tại
giường bệnh và đối chiếu với thông tin trên phiếu truyền máu
a) Khi truyền máu toàn phần, khối hồng cầu, khối bạch cầu: sử dụng huyết thanh mẫu định nhóm máu ABO của mẫu máu người bệnh được lấy ngay trước khi truyền máu và của mẫu máu lấy từ đơn vị máu sắp truyền.
b) Khi truyền tiểu cầu, huyết tương:
- Sử dụng huyết thanh mẫu định nhóm máu ABO mẫu máu của người bệnh;
- Trộn 02 giọt chế phẩm máu với 01 giọt máu người bệnh và kiểm tra ngưng kết. Không thực hiện truyền máu khi có ngưng kết, trừ trường hợp truyền tủa lạnh theo quy định tại Khoản 3 Điều 44 Thông tư này.
c) Phối hợp với đơn vị phát máu ,điều tra làm rõ mọi sự khác biệt (nếu có) giữa hồ sơ bệnh án, phiếu truyền máu, nhãn đơn vị máu và kết quả định nhóm.
4. Thực hiện việc truyền máu, theo dõi diễn biến, phát hiện, xử trí các bất thường về tình trạng sức khỏe của người bệnh:
a) Kiểm tra, theo dõi các chỉ số mạch, nhiệt độ, huyết áp, trạng thái tinh thần của người bệnh vào các thời điểm trước và trong quá trình truyền máu, đặc biệt lưu ý theo dõi trong 15 phút đầu truyền máu để phát hiện và xử trí kịp thời tai biên liên quan đến truyền máu;
b) Phải sử dụng bộ dây truyền máu có bầu lọc để truyền cho người bệnh;
c) Ghi hồ sơ đầy đủ các chỉ số mạch, nhiệt độ, huyết áp, trạng thái tinh thần, diễn biến lâm sàng của người bệnh, các xử trí (nếu có) vào phiếu truyền máu theo quy định tại Phụ lục 10 ban hành kèm theo Thông tư này;
d) Căn cứ tình trạng của người bệnh và diễn biến trong quá trình truyền máu, bác sỹ điều trị chỉ định việc theo dõi sau khi kết thúc truyền máu.
5. Không được bổ sung bất cứ chất gì (bao gồm cả các loại thuốc) vào túi máu, trừ trường hợp có chỉ định hoà loãng khối hồng cầu thì chỉ được sử dụng dung dịch muối đẳng trương (NaCl 0,9%) loại truyền tĩnh mạch.
6. Khi xảy ra các tai biến liên quan đến truyền máu, cơ sở điều trị phải thực hiện ngay các việc sau:
