Tôi tên là Nguyễn Ngọc Đoan Trang, công dân Việt Nam. Thường trú tại 146/59/18/2B đường Vũ Tùng, phường 2, quận Bình Thạnh, thành phố Hồ Chí Minh và đang tạm trú tại số 16/14A đường số 5, phường Hiệp Bình Chánh, quận Thủ Đức.
Từ ngày 06/10/2001 tôi sống chung với ông Hồ Văn Trung, quốc tịch Hoa Kỳ, là vợ chông nhưng không đăng kí kết hôn. Trong thời gian chung sống, chúng tôi có ba con gái có tên là Hồ Ngọc Đoan Thanh, Hồ Ngọc Thanh Thư, Hồ Ngọc Thanh Trúc. Các con tôi được sinh lần lượt vào các năm 2002, 2006, 2012 và tất cả các cháu đều có quốc tịch Hoa Kì.
Ngày 26 tháng 8 năm 2014, ông Trung đưa hai con gái của tôi là Hồ Ngọc Đoan Thanh và Hồ Ngọc Thanh Thư sang Hoa Kì để đi học và mang theo các giấy tờ cá nhân của các con tôi như Sổ an sinh xã hội, Giấy khai sinh, Học bạ. Tôi không biết ông ấy đã làm gì tại Hoa Kỳ, nhưng tôi chỉ biết một điều rằng ông ta đã không cho con tôi đi học như một người cha thường sẽ làm.
Sau đó, đến ngày 29 tháng 9 năm 2014, ông ấy lại đưa hai con gái của tôi về lại Việt Nam mà không có lý do gì cả. Hơn nữa, ông ta không cho các con tôi đi học. Là mẹ, tôi không thể khoanh tay đứng nhìn, tôi đã đưa cả ba con của tôi về sống tại địa chỉ mà đã nêu trên đã nhiều tháng nay.
Để chăm sóc và cho các con đi học, tôi đã yêu cầu ông Trung trợ cấp cho con của tôi và trả lại các giấy tờ trên. Ông ấy đồng ý trợ cấp cho cả ba đứa con của tôi mỗi tháng 10.000.000 đồng, nhưng lại không giao lại các giấy tờ trên cho tôi. Vì vậy tôi không thể làm thủ tục nhập học cho các con. Hơn nữa, số tiền trợ cấp của ông ấy quá ít, không đủ để tôi lo chi phí cho cuộc sống thường ngày, vì riêng tiền thuê nhà đã hết 5.000.000 đồng một tháng trong khi tôi lại là người thất nghiệp chỉ biết làm công việc nhà.
Để giải quyết các vấn đề trên, tôi đã cố gắng liên lạc với ông Trung ,nhưng không gặp, ngay cả khi tôi tìm tới nơi cư ngụ hiện giờ của ông mà tôi biết, tại 137 đường Xô Viết Nghệ Tĩnh, phường 17, quận Bình Thạnh.
Hiện tại, rất khó khăn cho tôi khi phải nuôi các con mà không đủ tiền và các giấy tờ cần thiết, tôi không có khả năng để thay đổi sự thật này. Do đó, tôi kính mong Tổng lãnh sự quán Hoa Kỳ có thể liên lạc với ông Trung, và yêu cầu ông ấy trả lại các giấy tờ mà tôi đã nêu trên để con tôi có thể tiếp tục đi học tại Việt Nam. Bên cạnh đó, vì những khó khăn về tài chính cũng như việc ông Trung hành xử như một người cha vô trách nhiệm, tôi mong Tổng lãnh sự quán Hoa Kỳ có thể cho tôi biết thông tin về các khoản trợ cấp mà ông Trung đang nhận được từ Chính phủ Hoa Kỳ. Từ đó tôi có thể dùng làm cơ sở đòi ông ấy thực hiện nghĩa vụ làm cha của mình hoặc bằng một cách nào đó Tổng lãnh sự quán Hoa Kỳ có thể cấp phần trợ cấp mà các con tôi đáng được hưởng cho các con tôi và khấu trừ vào phần trợ cấp của, ông Trung.
Cuối cùng, tôi xin phép được hỏi về quyền lợi của các con tôi. Vì cả ba đều là công dân chưa thành niên của Hoa Kỳ, các con của tôi có được hưởng bất cứ trợ cấp gì hay không? Nếu có, tôi cần phải làm những thủ tục gì để nhận được các khoản trợ cấp đó?
