1. Đề nghị Doanh nghiệp kiểm tra lại các nội dung sau:
Theo quy định tại Thông tư 50/2014/TT-BNNPTNT,- Phiếu kết quả thử nghiệm các chỉ tiêu chất lượng và vệ sinh an toàn thức ăn chăn nuôi của nhà sản xuất đã có một trong các giấy chứng nhận GMP, HACCP, ISO hoặc giấy chứng nhận tương đương. Trường hợp nhà sản xuất không có một trong những giấy chứng nhận GMP, HACCP, ISO hoặc giấy chứng nhận tương đương thì phiếu kết quả thử nghiệm phải được cấp từ các phòng thử nghiệm được cơ quan có thẩm quyền của nước xuất xứ chỉ định hoặc từ các phòng thử nghiệm được tổ chức công nhận quốc tế.
Đối với trường hợp của Công ty sản xuất (Nutriquest), nếu công ty đã có các chứng nhận ISO và HACCP, công ty nộp giấy chứng nhận phân tích của Công ty (hiện chứng nhận Iso mà công ty nộp cấp cho công ty Ridley Inc, đây không phải chứng nhận của nhà sản xuất (Nutriquest)).
2. Doanh nghiệp cần làm rõ và thông nhất các nội dung sau:
Đề nghị Doanh nghiệp làm rõ bản chất cụ thể của sản phẩm, mục đích sử dụng sản phẩm là bổ sung protein hay bổ sung nấm men ?. Theo công bố của nhà sản xuất Protein trong sản phẩm mức tối thiểu là 15 %, liều lượng sử dụng là 1,0 kg/tấn thức ăn, nếu với liều lượng như trên thì protein đóng góp trong khẩu phần là 0,15 % giá trị về dinh dưỡng (protein) cho vật nuôi, Như vậy, nếu sản phẩm chất chính là Protein thì việc bổ sung hàm lượng thấp như vậy sẽ không có tác dụng đáng kể trong việc bổ sung Protein cho vật nuôi.
3. Nộp lại giấy chứng nhận phân tích (Certificate of analysis) trong đó có đủ các thông tin sau:
- Tên và địa chỉ nhà sản xuất.
- Tên thương mại sản phẩm.
- Ngày sản xuất/lô sản xuất.
- Ngày phân tích/ ngày có kết quả phân tích.
- Kết quả phân tích:
+ Đối với các chỉ tiêu chất lượng: Ghi rõ các chỉ tiêu chất lượng quyết định bản chất của sản phẩm, trong đó ghi rõ tại mục đặc tính kỹ thuật của sản phẩm (specification) như: Mô tả cảm quan: Dạng, màu, mùi….; Độ ẩm hình thức công bố không lớn hơn (max); Xơ thô hình thức công bố tối đa (max); Đối với nấm men hình thức công bố mức tối thiểu (min), đơn vị tính CFU/g hoặc CFU/kg; kết quả phân tích; tên phương pháp phân tích, đơn vị tính.
+ Đối với các chỉ tiêu vệ sinh: Công bố hàm lượng tối đa cho phép đối với các vi sinh vật có hại (E. coli, Salmonella...), chỉ tiêu kim loại nặng (As, Cd, Pb, Hg...), trong đó ghi rõ tại mục đặc tính của sản phẩm (specification) mức giới hạn cho phép; kết quả phân tích; tên phương pháp phân tích; đơn vị tính.
+ Tên và chữ ký người chịu trách nhiệm về kết quả phân tích, có dấu xác nhận của đơn vị phân tích.
4. Nộp lại bản thông tin sản phẩm, trong đó có bổ sung các dung sau:
- Tên thương mại sản phẩm:
- Tên và địa chỉ nhà sản xuất:
- Thành phần nguyên liệu: Liệt kê tên các nguyên liệu cấu tạo thành sản phẩm (kể cả tên các chất phụ gia và chất mang)
- Mô tả cảm quan: Dạng, màu, mùi….
- Các chỉ tiêu chất lượng: Độ ẩm hình thức công bố không lớn hơn (max); Độ ẩm hình thức công bố không lớn hơn (max); Xơ thô hình thức công bố tối đa (max); Đối với nấm men hình thức công bố mức tối thiểu (min), đơn vị tính CFU/g hoặc CFU/kg.
- Các chỉ tiêu vệ sinh: Thông tin về một số chỉ tiêu vệ sinh trong sản phẩm như hàm lượng tối đa cho phép đối với các vi sinh vật có hại (E. coli, Salmonella...), chỉ tiêu kim loại nặng (As, Pb, Hg, Cd...) mà có nguy cơ nhiễm trong nguyên liệu và trong quá trình sản xuất sản phẩm.
- Công dụng:
- Hướng dẫn sử dụng: Ghi chi tiết về hướng dẫn sử dụng cho từng đối tượng vật nuôi (lợn, gà..).
- Hướng dẫn bảo quản và các cảnh báo khác (nếu có):
5. Nộp nhãn của sản phẩm, trong đó bổ sung sửa đổi:
- Đối với nhãn gốc: + Bổ sung mục thành phần nguyên liệu (Ingredients): Liệt kê tên các nguyên liệu cấu thành sản phẩm (kể các tên các chất phụ gia và chất mang)
(Liệt kê tên các thành phần nguyên liệu giống bản thông tin sản phẩm đã sửa).
+ Bổ sung chỉ tiêu chất lượng trong sản phẩm (Liệt kê tên các chỉ tiêu chất lượng giống bản thông tin sản phẩm đã sửa).
+ Công dụng:
+ Ghi chi tiết về hướng dẫn sử dụng: Ghi rõ hướng dẫn sử dụng cho đối tượng vật nuôi (lợn, gà…).
+ Tên và địa chỉ Công ty sản xuất.
- Đối với nhãn tiếng Việt: Ngoài nội dung dịch đúng nội dung dịch từ nhãn gốc bổ sung tên và địa chỉ Công ty nhập khẩu.
6. Nộp lại bản TCCS trong đó có sửa đổi bổ sung: Mục 3.2 Chỉ tiêu chất lượng (Liệt kê tên các chỉ tiêu chất lượng giống bản thông tin sản phẩm đã sửa)
Lưu ý: Tài liệu bổ sung là bản chính (có dấu xác nhận của nhà sản xuất) hoặc bản sao có chứng thực (không nộp bản sao y của Công ty nhập khẩu) và bản dịch ra tiếng Việt, tài liệu dịch ra tiếng Việt có xác nhận của Công ty nhập khẩu.
