Bản Thỏa thuận chấm dứt Hợp đồng lao động này (Sau đây gọi tắt là “Thỏa Thuận”) được lập ngày 02/03/2015 bởi Các Bên:

Hai Bên thỏa thuận và thống nhất nội dung sau:
Điều 1: Chấm dứt Hợp đồng lao động
1.1 CÔNG TY TNHH DICENTRAL VIỆT NAM (tên cũ là Công ty TNHH Trung Tâm Di) và Người Lao Động đồng ý chấm dứt Hợp đồng lao động số 111/2012/HĐLĐ – DIC ký ngày 29/09/2012.
1.2 Bên A đồng ý sẽ hỗ trợ cho bên B thêm một khoản tiền tương đương là 28,500,000VND (Hai mươi tám triệu năm tram nghìn đồng chẵn) cho việc hai bên chấm dứt hợp đồng này.
1.3 CÔNG TY TNHH DICENTRAL VIỆT NAM có trách nhiệm sẽ thanh toán đầy đủ tiền lương tính đến ngày chấm dứt hợp đồng (02/03/2015), trợ cấp thôi việc, trợ cấp mất việc làm theo đúng quy định của luật Lao động hiện hành ngay khi bên B bàn giao các trang thiết bị mà bên A đã cung cấp cho bên B trong quá trình làm việc
Điều 2: Thỏa thuận khác
2.1 Thỏa Thuận này được lập thành hai (2) bản tiếng Việt có giá trị như nhau. Mỗi Bên giữ một bản để thực hiện.
2.2 Các Bên đồng ý rằng, bằng sự hiểu biết tốt nhất của mình, đã đọc, hiểu và đồng ý chịu sự ràng buộc bởi các điều khoản của Thoả thuận này.
TRƯỚC SỰ CHỨNG KIẾN CỦA HAI BÊN, đại diện có thẩm quyền của HAI BÊN đã ký vào Thỏa Thuận này vào ngày được ghi ở trên.
