Khi tiến hành mở mã đơn vị tai BHXH quận 2, đơn vị được BHXH quận 2 đề xuất lấy tên của công ty mẹ đã được đăng ký tại BHXH thành phố là “CÔNG TY CDM INTERNATIONAL INC” thay vì tên của đơn vị được thể hiện trong Quyết định thành lập số 129/QĐ-BXD ngày 9 tháng 12 năm 2014 là “Thầu chính gói thầu CS5 – Thiết kế chi tiết và giám sát thi công thuộc Dự án Cấp nước thành phố Hồ Chí Minh theo khoản vay tín dụng ADB số 2754-VIE”.
Tuy nhiên, tên của đơn vị đăng ký tại BHXH quận 2 và tên của đơn vị trong Quyết định thành lập nêu trên cũng không trùng với tên chủ tài khoản (tên đơn vị thụ hưởng) mở tại ngân hàng Sacombank chi nhánh Phan Đăng Lưu. Do đó, bằng công văn này, đơn vị chúng tôi xin xác nhận lại thông tin tài khoản của đơn vị theo như nội dung nêu trên.
