PHỤ LỤC I: MỘT SỐ LƯU Ý KHI THỰC HIỆN KHÁM SỨC KHỎE
1. LẤY MÁU
- Cần nhịn ăn, và không uống các chất kích thích như cà phê, trà, nước uống có ga, bia rượu… ít nhất 6 giờ trước khi lấy mẫu máu. Duy nhất có thể uống nhiều nước lọc.
- Đăng ký danh mục xét nghiệm làm thêm chỉ thực hiện khi nhân viên khám tại bệnh viện (Do nhân viên tự thanh toán với bệnh viện)

2. HƯỚNG DẪN CÁCH LẤY NƯỚC TIỂU
- Bước 1: Vệ sinh phần sinh dục ngoài
- Bước 2 : Bỏ lượng nước tiểu đầu, sau đó hứng tiếp khoảng ½ lọ. Đậy kín nắp, mang đến phòng xét nghiệm

- CHÚ Ý: đối với phụ nữ không lấy mẫu nước tiểu trong những ngày hành kinh và 2 ngày sau đó.( NẾU KHÔNG LẤY ĐƯỢC MẪU NƯỚC TIỂU VUI LÒNG BÁO CHO KỸ THUẬT VIÊN)

3. KHÁM LÂM SÀNG
- Địa điểm: Tầng M, 60 – 60A Phan xích Long, P1, Q. Phú nhuận
- Nữ mang thai không chụp X- Quang
- Nữ siêu âm trước khi chụp X- Quang.
- Vui lòng báo cho điều dưỡng những hạng mục mà KH không đồng ý thực hiện.
- Uống nhiều nước, nhịn tiểu trước khi siêu âm bụng.
- Vui lòng mặc quần áo, giầy dép thoải mái, tránh mặc váy, vì bạn có thể được yêu cầu thay quần, áo của bệnh viện.
- Vui lòng liên hệ điều dưỡng - Quầy tiếp nhận tầng M

Ghi chú:
- Đối với các công ty có đăng ký thêm các mục “Siêu âm đầu dò âm đạo” và “Đo loãng xương, Phế dung ký” sẽ thực hiện khám ở Tầng 3.
- Nếu gói khám trong hợp đồng công ty không có các mục khám như thứ tự hướng dẫn bên trên thì không cần thực hiện.
Diễn giải:
- Chăm sóc khách hàng hướng dẫn khách hàng đến tầng M để thực hiện khám sức khỏe.
- B1: Tại quầy tiếp nhận tầng M, khách hàng cung cấp thông tin cá nhân cho điều dưỡng nhận bệnh (Họ tên, năm sinh, Tên công ty,…)
- Điều dưỡng tại quầy tiếp nhận tầng M sẽ phát sổ kiểm tra sức khỏe định kỳ theo Thông tư 14 của Bộ y tế.
- B2: Điều dưỡng (hoặc chăm sóc khách hàng – trong trường hợp công ty đông nhân viên) sẽ thực hiện Cân đo cân nặng, chiều cao, huyết áp cho khách hàng.
- B3: Kỹ thuật viên phòng Xét nghiệm chịu trách nhiệm hướng dẫn và lấy mẫu máu cho khách hàng tại tầng M.
- B4-5-6-7: Điều dưỡng tại quầy tiếp nhận Tầng M hướng dẫn khách hàng nơi thực hiện các danh mục: Khám Nội -ngoại-da liễu -phụ khoa; Khám Tai Mũi Họng, Mắt, Răng Hàm Mặt; Siêu âm bụng -Tuyến giáp- Tuyến vú; Siêu âm tim -Điện tâm đồ.
- B8: Điều dưỡng Tầng M (hoặc chăm sóc khách hàng) hướng dẫn khách hàng lên tầng 1 để chụp film X-Quang.
- B9: Khách hàng sẽ trả hồ sơ kiểm tra sức khỏe tại quầy tiếp nhận Tầng M.
