Giấy “Đăng ký kiểm tra nhà nước về chất lượng hàng hóa nhập khẩu” theo mẫu.
Bản sao chứng chỉ chất lượng có chứng thực.
Tài liệu kỹ thuật khác liên quan: Bản sao (có xác nhận của người nhập khẩu)
Bill of Lading;
Invoice
tờ khai hàng hóa nhập khẩu;
giấy chứng nhận xuất xứ (nếu có) (C/O-Certificate of Origin);
ảnh hoặc bản mô tả hàng hoá;
mẫu nhãn hàng nhập khẩu đã được gắn dấu hợp quy và nhãn phụ (nếu nhãn chính chưa đủ nội dung theo quy định
Bản sao hợp đồng mua bán và danh mục hàng hóa kèm theo hợp đồng
– Sau khi cơ quan tiếp nhận hồ sơ sẽ kiểm tra tính hợp lệ và đầy đủ , trường hợp đầy đủ thì tiếp nhận hồ sơ và tiến hành kiểm tra theo quy định, trường hợp khồng đầy đủ thì sẽ thông báo bổ sung cho người đăng ký

– Trong 3 ngày sẽ tiến hành kiểm tra, đánh giá kết quả và gửi trả hồ sơ
