 Tiến hành đánh giá:
- Sử dụng các phương pháp (phỏng vấn, quan sát các hoạt động, xem xét tài liệu, chụp ảnh, ghi âm,…) để thu thập các bằng chứng về việc thực hiện và duy trì Hệ thống tại đơn vị được đánh giá, kiểm tra việc khắc phục các điểm không phù hợp của lần đánh giá trước.
- Trao đổi trực tiếp với bên được đánh giá về các phát hiện, giải thích rõ cho bên được đánh giá về căn cứ để xác định các điểm không phù hợp, các điểm khuyến nghị. Tất cả các điểm được xác định là điểm không phù hợp, điểm khuyến nghị phải được sự thống nhất và đồng ý của đơn vị được đánh giá.
- Đánh giá viên cần báo cáo kịp thời cho trưởng đoàn đánh giá biết các vướng mắc phát sinh trong quá trình đánh giá. Trưởng đoàn có trách nhiệm thay mặt đoàn đánh giá giải quyết các vướng mắc đó với các bên liên quan.
- Các nội dung đánh giá được ghi chép theo mẫu BSR-PRO-QHSE-003/F-004.
- Căn cứ vào các định nghĩa phân loại các điểm phát hiện nêu ở mục 4, các đánh giá viên có trách nhiệm viết báo cáo rõ ràng, ngắn gọn về các phát hiện, bằng chứng về các điểm không phù hợp, điểm khuyến nghị đã phát hiện được cho trưởng đoàn theo biểu mẫu BSR-PRO-QHSE-003/F-003 và thống nhất với trưởng đoàn về các điểm đó.
- Trưởng đoàn đánh giá có trách nhiệm tổng hợp, hoàn thiện Báo cáo đánh giá nội bộ cho toàn đoàn theo biểu mẫu BSR-PRO-QHSE-003/F-003. Báo cáo này được lập theo từng đơn vị được đánh giá.
- Sau khi kết thúc đánh giá, các Trưởng đoàn đánh giá và lãnh đạo Ban ISO (chủ trì) họp để xem xét, thống nhất các điểm phát hiện trong đợt đánh giá trước cuộc họp kết thúc.
