Chúng tôi xin trình bày sự việc ngày 25/10/2016
Chúng tôi gồm:
 TRỊNH HỮU KHÁNH (Giám sát kỹ thuật-nhà thầu LLM18E)
 VŨ QUANG HIỀN (Lái xe cẩu tự hành-nhà thầu LMM18E)


Ngày 25/10/2016 vào lúc 9h00’ chúng tôi lấy tủ Remote cabinet No.2 tại Laydown1 bằng xe cẩu tự hành mang biển kiểm soát :51C-607.04 do anh VŨ QUANG HIỀN điểu khiển với sự chứng kiến của MC.Tủ được đựng trong thùng gỗ ,lúc cẩu kéo lên xe theo đúng quy trình kỹ thuật và an toàn, không có bất kỳ sự va đập, rơi đổ nào trong quá trình cẩu hàng.Trước và sau khi cẩu tủ lên xe không có dấu hiệu nào bất thường của thùng gỗ chứa tủ.Sau đó thùng gỗ chứa tủ được đưa về tầng trệt của nhà Coal handing,Quá trình vận chuyển tủ diễn ra bình thường.
Lúc 15h:00’ ngày 25/10/2016 với sự chứng kiến kiến của JEL, MC chúng tôi tiến hành mở thùng gỗ chứa tủ Remote Cabinet No.2 với những dụng cụ chuyên dụng và không có sự va chạm giữ dụng cụ và thiết bị trong thùng gỗ.Sau khi mở vỏ gỗ bên ngoài thì thấy lớp bọc túi nilong tráng bạc,không có dấu hiệu bất thường nào bên ngoài.Tiếp đến chúng tôi tiếp tục cắt lớp túi nilong tráng bạc.Sau khi cắt lớp Nilon tráng bạc bọc bên ngoài tủ ra thì chúng tôi thấy tủ Remote Cabinet No.2 có nhiều vấn đề hư hại như sau:
 Giá đỡ bình acquy bị sập nghiêng 1 góc 30°,bình acquy bị nghiêng , đổ về phía cánh tủ
 Thiết bị Radio bị rơi xuống sát đáy tủ
 Thiết bi bên trong tủ bị lệch khỏi vị trí
 Tấm đỡ bị cong,xước bề mặt sơn một số vị trí
Khi phát hiện tủ có nhiều vấn đề như vậy chúng tôi có ghi lại hình ảnh và có giám sát JeL,MC xác nhận.Sau đó với sự đồng ý của MC chúng tôi đặt bình acquy về lại vị trí cân bằng,khóa tủ,che đậy cận thận.
