- Danh mục hàng hóa kèm theo

3. Một hoặc các bản sao chứng chỉ chất lượng (có chứng thực)
4. Các tài liệu có liên quan:

- Bản sao (có xác nhận của người nhập khẩu) vận đơn;

- Hóa đơn;

- Tờ khai hàng hóa nhập khẩu;

- Giấy chứng nhận xuất xứ (nếu có);

- Ảnh hoặc bản mô tả hàng hóa; mẫu nhãn hàng nhập khẩu đã được gắn dấu hợp quy và nhãn phụ nếu nhãn chính chưa đủ nội dung theo quy định
