Giải thích biểu mẫu:

(1) Ghi họ tên, chức danh của cán bộ tham mưu, đề xuất việc giải quyết.
Ví dụ: ông/bà Nguyễn Văn A, Viên chức lãnh sự
(2) Ghi theo loại việc hộ tịch thực tế mà đương sự yêu cầu ghi vào sổ: “kết hôn” hoặc “nhận cha, mẹ, con”
(3) Ghi họ và tên của người có yêu cầu
Ví dụ: về việc ghi vào sổ hộ tịch việc kết hôn của ông Hoàng Văn H.
(4) Ghi nơi sinh theo 2 cấp đơn vị hành chính (cấp tỉnh, tên nước)
Ví dụ: thành phố Hải Phòng, Việt Nam
hoặc: Paris, Cộng hoà Pháp
(5) Ghi rõ tên loại, số của giấy tờ đương sự nộp trong hồ sơ
Ví dụ: Hộ chiếu số N1234568
Giấy phép tái nhập cảnh số GP 34567
Thẻ cư trú số #787878
(6) Ghi tên loại sổ hộ tịch thực hiện ghi vào sổ: “kết hôn” hoặc “nhận cha, mẹ, con”, số thứ tự đăng ký trong sổ và số quyển của sổ.
(7) Ghi ngày thực hiện việc ghi vào sổ hộ tịch
Ví dụ: Đã thực hiện ghi vào Sổ đăng ký kết hôn, số 256, quyển số 02, ngày 20 tháng 02 năm 2015.
(8) Ghi rõ loại việc, các thông tin về nhân thân của vợ/chồng, người được nhận là cha/mẹ/con (nếu có), bao gồm: họ và tên, ngày, tháng, năm sinh, dân tộc, quốc tịch, tên loại và số Hộ chiếu/Giấy CMND/giấy tờ khác chứng minh về nhân thân, nơi cư trú.
Ví dụ: Việc kết hôn với bà Nguyễn Thị V., sinh ngày 15/5/1995, dân tộc: Kinh, quốc tịch: Hoa Kỳ, Hộ chiếu số VN1256789, cư trú tại New York, Hoa Kỳ.
hoặc: Việc nhận con là cháu Hoàng Hữu K., sinh ngày 01/01/2014, dân tộc: Kinh, quốc tịch: Việt Nam, Giấy khai sinh số 01/2014, quyển số 02/2013, UBND xã …, thành phố Thanh Hoá, tỉnh Thanh Hoá cấp ngày 15/01/2014.
(9) Ghi rõ tên loại, số, cơ quan cấp, ngày cấp văn bản là căn cứ thực hiện việc ghi vào sổ hộ tịch.
Ví dụ: Giấy chứng nhận kết hôn số 56789, do Phòng hộ tịch tỉnh Québec, Canada cấp ngày 14/02/2014.
