Trước hết em xin nhận khuyết điểm vì đã để xảy ra sự cố này. Sau đây em xin đươc trình bày để Anh được rõ:
1/ Khi nhận ca, em đã đuợc ca trước giao lại là ST product cho APT đã hết, lúc đó mức các Tank BA731/732 là 2.3m. Em đã báo cáo Mr Alan khi họp giao ban và Mr Alan bảo đợi Mr Quân đến, khi đó em đã nhấn mạnh là ST đã hết mà Mr Quân thì 8h mới đến. Đúng lúc đó thì Mr Damian bước vào, em đã bảo em luyện phiên dịch nói với Mr Damian là ST đã hết và yêu cầu cấp thêm, Mr Damian bảo sau cuộc họp thì sang phòng tuyển khoáng để trao đổi.
2/ Sau cuộc họp em sang phòng tuyển khoáng Mr Damian bảo em chạy lại một máy nghiền bằng nước mềm sau đó lại bảo chạy hai máy nghiền và không nói gì thêm. Em nghĩ đó là giải pháp và đi chuẩn bị chạy lại hai máy nghiền.
3/ Khoảng 11h Mr Dũng sang gặp em nói là Anh ơi sắp hết dung dịch cho APT rồi, em bảo đã thông báo cho Mr Damian nhưng giờ vẫn chưa thấy đến.
Trên đây là toàn bộ sự việc.
Em xin chân thành nhận khuyết điểm và coi đây là bài học đắt giá để nêu cao tinh thần hơn nữa không để sự cố tương tự xảy ra.
