Đảm bảo kết quả xét nghiệm được chính xác và an toàn cho người bệnh trong quá trình lấy mẫu, xử lý, phân tích và trả kết quả xét nghiệm tại Khoa Huyết học truyền máu.

- Công văn số 2600-SYT-HĐQLCLKCB ngày 13 tháng 5 năm 2014 của Sở Y tế ban hành khuyến cáo triển khai các hoạt động an toàn người bệnh tại các bệnh viện;
- Quyết định số 56/BYT-K2ĐT ngày 16 tháng 5 năm 2014 của Bộ Y tế ban hành tài liệu đào tạo liên tục An toàn người bệnh.
7. NỘI DUNG
Đảm bảo an toàn người bệnh trong công tác kiểm soát nhiễm khuẩn:
- Thực hiện vệ sinh tay: Có đủ phương tiện vệ sinh tay, rửa tay đúng thời điểm, giám sát, kiểm tra dựa vào bảng kiểm;
- Phân loại rác đúng quy định;
- Thực hiện xử lý dụng cụ ban đầu đúng quy trình thích hợp.
An toàn người bệnh trong quy trình chuyên môn:
- Thực hiện tốt quy trình chuyên môn, kiểm tra, giám sát dựa vào bảng kiểm, nhất là các quy trình: Quy trình lấy máu, quy trình định nhóm máu, quy trình xét nghiệm hòa hợp, quy trình cấp phát máu và chế phẩm máu…
- Thường xuyên kiểm tra, bảo trì, hiệu chỉnh máy móc.
- Chống nhầm lẫn:
+ Nhầm tên người bệnh: Kiểm tra tên với người bệnh: Không nói tên người bệnh để họ xác nhận lại mà yêu cầu người bệnh tự nói tên của họ, đối với trẻ nhỏ cần có xác nận của người lớn đi kèm.
+ Nhầm bệnh phẩm, chỉ định:
• Kiểm tra thông tin người bệnh với bệnh phẩm: ID, tên, tuổi…
• Sử dụng ít nhất 2 công cụ nhận dạng người bệnh (Tên, tuổi, ID…) cho việc lấy mẫu xét nghiệm hoặc để cung cấp các sản phẩm về máu và sử dụng cả 2 công cụ nhận dạng để dán nhãn hoặc ghi cho các vật chứa mẫu, đảm bảo công cụ nhận dạng vẫn giữ trên các mẫu trong suốt quá trình trước phân tích, trong phân tích và sau phân tích.
- Việc nhận y lệnh và trả kết quả qua điện thoại: Không nhận y lệnh và trả kết quả qua điện thoại trừ trường hợp hết sức cần thiết.
+ Người nhận y lệnh phải viết ra và đọc lại đúng nguyên văn cho bác sỹ đã ra y lệnh nghe, sau đó bác sỹ này xác nhận y lệnh là chính xác;
+ Đối với việc trả lời kết quả bằng miệng hoặc qua điện thoại: Yêu cầu nhân viên nhận đọc lại kết quả xét nghiệm.
- Tất cả các kết quả, gía trị xét nghiệm quan trọng phải được báo cáo trực tiếp cho nhân viên chịu trách nhiệm, nếu nhân viên trách nhiệm không có mặt ở đó lúc làm việc thì báo cho nhân viên trực, sau đó báo lại cho nhân viên chịu trách nhiệm.
- Phải có sự bàn giao đầy đủ khi giao ca.
Giảm nguy cơ trượt ngã:
- Kiểm tra sự an toàn ở lan can, tay vịn, ghế… nếu có hư hỏng báo cho Phòng Hánh chánh quản trị để sửa chữa, thay thế.
- Người bệnh là trẻ em phải có người lớn đi cùng.
- Có bảng cảnh báo vấp ngã.
Phát hiện sự cố và sai sót:
- Khi có sự cố phải báo cáo, phân tích nguyên nhân, đề xuất, tiến hành các hoạt động chủ động giảm bớt rủi ro.
