Trong thời gian công tác tại Công ty với nỗ lực bản thân cùng với sự giúp đỡ, chỉ đạo sát sao của Lãnh đạo Công ty, lãnh đạo Phòng, sự phối hợp chặt chẽ của các đơn vị trực thuộc Công ty và sự ủng hộ, tín nhiệm của các cán bộ thuộc Phòng Thiết kế, tôi đã hoàn thành các nhiệm vụ được giao, góp phần tích cực vào kết quả hoạt động của Phòng và của Công ty.
Trong suốt quá trình công tác bản thân tôi ngoài việc hoàn thành công việc, hợp tác tốt với đồng nghiệp và không mắc khuyết điểm nào, tôi cũng nhận thấy công việc hiện tại phù hợp với trình độ chuyên môn và năng lực của tôi; nay do hoàn cảnh gia đình tôi có mong muốn xin nghỉ việc. Vì vậy, tôi làm đơn này kính đề nghị lãnh đạo Công ty, Phòng Tổ chức Hành chính, Trưởng phòng Thiết kế cho phép và giúp làm các thủ tục cần thiết để tôi được chấm dứt hợp đồng kể từ ngày 01 tháng 4 năm 2015
