Sau khi đóng hàng vào container xong, tài xế cho đầu xe lùi để ráp mooc kéo container ra khỏi vị trí máng đóng hàng.

Tuy nhiên khi thực hiện ráp mooc và chuẩn bị kéo ra khỏi máng đóng hàng thì khóa mâm đĩa của đầu kéo bị trượt ra khỏi trục gắn của mooc. Việc này làm mooc bị đẩy lùi ra phía sau và kê lên cầu xuống hàng của Kho Nitto
