Căn cứ vào công văn ngày 29 tháng 4 năm 2016 của thành viên liên danh nhà thầu Công ty

Establissement A Menart SPRL về việc đệ trình thayđổi một số chi tiết trong dây truyền tái chế nhựa.

Căn cứ vào thư trả lời số: 11 99-TTCST/160513_2/MP ngày 13.05.2016 về việc yêu cầu nhà

thầu giải trình các thay đổi trong dây truyền tái chế nhựa.

Căn cứ vào cuộc họp và buổi kiểm tra công trường giữa Tư vấn giám sát, nhà thầu và Chủ đầu

tư. Trong đó Tư vấn giám sát yêu cầu nhà thầu thay thế và chỉnh sửa các chi tiết trong dây chuyền tái

chế nhựa.

Nhà thầu xin đệ trình giải thích về sự thay đổi trong dây truyền tái chế nhựa để ông xem sét

và phê duyệt.
