Nay, tôi làm công văn này gởi đến phòng xuất TpHCM để xin xác nhận về thời gian nhập cảnh và xuất cảnh tại Việt Nam từ tháng 02 năm 2014 đến tháng 08 năm 2014 của cuốn hộ chiếu S05338530.
Lý do: Hiện nay tôi đang làm hồ sơ quyết toán thuế thu nhập cá nhân. Trong hồ sơ quyết toán có bảng kê số ngày cư trú tại Việt Nam phải dựa vào Passport để xác định cá nhân cư trú hay không cư trú nhưng do hộ chiếu số S05338530 của tôi đã thu hồi. Vì hộ chiếu S05338530 là loại hộ chiếu tại Hàn Quốc quy định chỉ xuất cảnh và nhập cảnh 1 lần nên muốn xuất cảnh thì tôi phải làm hộ chiếu khác. Và hộ chiếu củ này sẽ bị tịch thu lại sau khi cấp hộ chiếu mới. Số hộ chiếu mới là M25716223 cấp ngày 20/08/2014.
Ngày nhập cảnh vào Việt Nam của hộ chiếu S05338530 là: 17/02/2014
Ngày xuất cảnh vào Việt Nam của hộ chiếu S05338530 là: 19/08/2014
Hiện nay, do không còn cuốn hộ chiếu trên nên tôi không thể có được số ngày nhập cảnh và xuất cảnh tại Việt Nam để hoàn thành hồ sơ quyết toán thuế cho cơ quan thuế Tp.HCM để hoàn thành nghĩa vụ nộp thuế thu nhập cá nhân.
Vậy nay tôi làm công văn này kính gởi lên Phòng xuất nhập cảnh Tp.HCM xin được xác nhận thời gian nhập cảnh và xuất cảnh để tôi có thể hoàn thành nghĩa vụ trên.

Reason: Currently I am records the settlement of personal income tax. In the profile list of settlements dates residing in Vietnam to rely on Passport to identify individual resident or non-resident, but due to my passport number S05338530 recovered. Because S05338530 passport is in Korean passport regulations only one exit and entry-exit times so I want to do another passport. And this old passport will be confiscated after the new passport. Number of new passports are issued on 20.08.2014 M25716223.
