1. MỤC ĐÍCH
Thực hiện giải quyết các sự cố về an toàn sinh học khi xảy ra một cách nhanh chóng, chính xác, hạn chế nguy cơ phơi nhiễm cho nhân viên y tế và cộng đồng.
2. PHẠM VI ÁP DỤNG
Áp dụng tại Khoa Huyết học truyền máu – Bệnh viện Quận Thủ Đức.

- Thông tư số 25/2012/TT-BYT ngày 29 thang 11 năm 2012 ban hành Quy chuẩn kỹ thuật quốc gia về thực hành và an toàn sinh học tại phòng xét nghiệm.
7. NỘI DUNG
Nguyên tắc xử lý sự cố an toàn sinh học:
- Xử lý tại chỗ;
- Lưu giữ hồ sơ sự cố;
- Báo cáo với người phụ trách Phòng xét nghiệm.
7.1. Xử lý sự cố kim đâm vào tay khi làm việc với tác nhân gây bệnh
Báo với đồng nghiệp làm việc gần đó (Nếu có);
Bộc lộ vết thương (Cởi hoặc xé găng tay);
Đưa ngay vết thương dưới vòi nước sạch;
Để vết thương tự chảy máu trong một thời gian ngắn, không nặn bóp vết thương;
Rửa khu vực vết thương kỹ bằng xà phòng và nước sạch;
Sử dụng băng gạc với chất khử nhiễm thích hợp để che vết thương;
Rời khỏi PXN như thường lệ;
Ghi chép và báo cáo với người phụ trách PXN;
Tùy từng trường hợp cụ thể có cách xử lý tiếp theo phù hợp.
7.2. Xử lý sự cố đổ, tràn dung dịch chứa tác nhân gây bệnh
Thay găng tay, đeo kính;
Dùng kẹp gắp dụng cụ đựng mẫu cho vào túi đựng chất thải lây nhiễm, vật sắc nhọn bỏ vào hộp đựng vật sắc nhọn;
Trải giấy thấm lên dung dịch bị đổ từ ngoài vào trong;
Đổ chất khử khuẩn lên vùng bị đổ theo chiều từ ngoài vào trong;
Để khoảng 30 phút cho chất khử nhiễm phát huy tác dụng diệt khuẩn;
Thay găng tay mới;
Lau sạch vùng bị đổ bằng giấy thấm và cho vào túi đựng chất thải lây nhiễm;
Ghi chép, báo cáo sự việc với cán bộ phụ trách PXN.
7.3. Xử lý sự cố đổ, vỡ ống chứa tác nhân gây bệnh trong máy ly tâm
Nếu MLT đang hoạt động thì tắt máy, vẫn đóng nắp, để 30 phút cho khí dung lắng xuống hết;
Nếu sau khi mở nắp MLT mới phát hiện đổ, vỡ: Ngay lập tức đóng nắp lại, đi ra khỏi phòng.
Báo với đồng nghiệp cùng rời khỏi PXN và đóng cửa lại;
Đặt dấu hiệu cảnh báo cho những người xung quanh ở cửa ra vào;
Đợi 30 phút để khí dung lắng xuống;
Mang mắt kiếng, găng tay, mắt kính vào xử lý sự cố;
Dùng giấy thấm có chất khử nhiễm thích hợp lau bên trong của MLT;
Dùng kẹp gắp mảnh vỡ ống nghiệm cho vào hộp đựng dụng cụ sắc nhọn;
Khử nhiễm cho rotor máy và cốc ly tâm bằng dung dịch thích hợp;
Ghi chép, báo cáo sự việc với cán bộ phụ trách PXN.
7.4. Xử lý sự cố hỏa hoạn
Tránh làm người bị thương;
Cảnh báo với đồng nghiệp để sơ tán;
Kích hoạt chuông báo cháy;
Đóng cửa;
Sơ tán đến nơi an toàn.
