QUYẾT ĐỊNH

Article 1: Chấm dứt hợp đồng lao động với ông/bà TAKIZAWA KAZUO kể từ ngày 31 tháng 07 năm 2016. Ông/bà TAKIZAWA KAZUO có trách nhiệm giải quyết các thiệt hại nếu có xảy ra và bàn giao công việc và tài sản, dụng cụ làm việc cho công ty trước khi thôi việc hẳn.
Article 2: Ông/bà TAKIZAWA KAZUO được hưởng các chế độ sau:
2.1 Tiền lương những ngày làm việc trong tháng 07/2016
2.2 Bảo hiểm y tế: công ty sẽ đóng đầy đủ cho người lao động đến khi quyết định này có hiệu lực..
Article 3: Công ty chỉ thanh toán chế độ điều 2.1 khi có bản bàn giao công việc.
Article 4: Ông/bà TAKIZAWA KAZUO và các bộ phận có liên quan có trách nhiệm thi hành quyết định này. Quyết định có hiệu lực kể từ ngày ký.
