Mẫu máu thiếu: Yêu cầu lấy lại mẫu máu khác, ghi nhận vào sổ sai sót chuyên môn.
Mẫu máu có kết quả bất thường:
- Kiểm tra lại ống máu (Xem ống máu có bị đông, tán huyết, lấy không đủ số lượng, lấy máu truyền dịch…);
- Xem lại kết quả cũ;
- Kiểm tra lại máy (Máy báo lỗi, máy dơ…), rửa máy, chuẩn lại máy (Nếu cần);
- Kiểm tra lại thuốc thử (Thuốc thử hết, nhiễm bụi bẩn, nghẹt đường ống dẫn, nhầm thuốc…), thay thuốc thử (Nếu cần);
- Chạy lại mẫu máu trên cùng máy và khác máy (Nếu có) để kiểm chứng;
- Kéo lam, nhuộm để kiểm chứng (Nếu là xét nghiệm tế bào);
- Nếu kết quả vẫn nghi ngờ, yêu cầu lấy máu lại, ghi nhận vào sổ sai sót chuyên môn.
Mẫu máu không có giấy chỉ định, không có/không đủ/ sai thông tin người bệnh, thông tin theo yêu cầu:
- Mẫu máu từ phòng lấy máu: Yêu cầu người lấy máu bổ sung chính xác thông tin người bệnh, ghi nhận vào sổ sai sót chuyên môn;
- Mẫu máu từ khoa/ phòng khác: Yêu cầu nhân viên có trách nhiệm của khoa/ phòng đó bổ sung chính xác thông tin người bệnh, bệnh phẩm, ghi nhận vào sổ sai sót chuyên môn và yêu cầu ký tên.

Kết quả (Anh) 1:

Blood sample lacks: request other blood samples back, recorded on the shared professional errors.Blood samples have abnormal results: -Check the blood tubes (see blood tubes have been East, hemolysis, get insufficient blood perfusion, number ...);-Review the old results;-Check the air (Air error message, your Venice ...), washing machine, standard air (If necessary);-Check that the reagents (reagent, contamination, stuffy, wrong pipe smoking...), instead of reagents (If necessary);-Restart the blood samples on the same air and the other (If any) to verify;-Pull the blue dye, to verify (if the cell tests);-If the results are still in doubt, require the taking of blood, recorded on the shared professional errors.Blood samples have no paper, no/insufficient/wrong patient information, information as required:-Blood samples from blood: asking the correct supplement blood patient information, recorded on the shared professional flaws;-Blood samples from other departments: staff requirements are the responsibility of the departments that supplement patient information accurately, swabs, recorded on the shared professional errors and require signature.

Blood samples missing: Request regain another blood sample, recorded in the book of medical errors.
Blood samples with abnormal results:
- Check your blood tubes (see pipes have frozen blood, hemolysis, taking insufficient number amount, taking blood perfusion ...);
- Review past results;
- Check your machine (machine error, the machine dirty ...), washing machine, recalibrate the machine (if required);
- Check your reagents (drugs try out, contamination, obstruction pipeline, wrong drugs ...), replace the reagents (if necessary);
- Rerun blood samples and other machine on the same machine (if any) to verify;
- Drag blue, dyed to verification (If the cytology);
- If the result is still in doubt, ask the blood collection, recorded in the book of medical errors.
the blood sample without specifying paper, no / insufficient / false information patient information, information on demand:
- blood samples taken from the blood room: to request the additional blood draw accurate patient information, recorded in the book of medical errors;
- blood samples from faculties / living room: require employees responsible departments / sections that additional patient information correctly, specimens, recorded in the book of medical errors and asked to sign.
