a. Lỗi/Sự cố do dịch vụ chưa tối ưu:
- Định nghĩa: Là các sự cố xảy ra do đang trong quá trình phát triển dịch vụ mới hoặc hệ thống mới chưa dự liệu được hết phát sinh xảy ra;
- Trách nhiệm: Tháng đầu tiên để xảy ra lỗi do chưa dự trù hết thì đơn vị chủ trì sẽ chịu trách nhiệm. Từ lần thứ 2 trở đi, lỗi nằm ở đơn vị nào sẽ do đơn vị đó chịu trách nhiệm.
b. Lỗi/Sự cố do để lọt lỗi:
- Định nghĩa: Là các lỗi/sự cố xảy ra do đơn vị kiểm thử chưa test kỹ các chức năng của hệ thống, phần mềm;
- Trách nhiệm: Lỗi liên quan đến đơn vị nào thì đơn vị đó chịu trách nhiệm.

a.
