Hai bên thống nhất những nội dung về việc phối hợp khám chữa bệnh bảo hiểm y tế từ ngày 01 tháng 04 năm 2015 đến hết ngày 31 tháng 12 năm 2015 :
Điều 1: Trách nhiệm của hai bên
Bên B: Công ty TNHH ChangShin VN
Có trách nhiệm thực hiện báo cáo bảo hiểm y tế hàng quý gửi về cho Bệnh viện ña khoa huyện Vĩnh Cửu theo đúng quy định của Bảo hiểm y tế Đồng Nai, gửi kịp thời cho bên A.
Có trách nhiệm sử dụng đúng thuốc do bên A cung cấp theo dự trù của bên B và kiểm kê gửi báo cáo nhập, xuất, tồn hàng tháng về cho bên A.
Thöïc hieän ñuùng theo caùc quy ñònh phaùp luaät khaùm chöõa beänh BHYT.
Bên A: Bệnh viện đa khoa huyện Vĩnh Cửu.
Có trách nhiệm thực hiện báo cáo bảo hiểm y tế hàng quý theo quy định của Bảo hiểm y tế Đồng Nai bao gồm cả báo cáo bảo hiểm y tế của bên B, gửi kịp thời về Bảo hiểm y tế Đồng Nai.
Có trách nhiệm cung cấp đủ thuốc cho bên B theo quy định, đồng thời không chịu trách nhiệm về thuốc không do bên A cung cấp.
Có trách nhiệm xuất hóa đơn theo đúng quy định cho Bảo hiểm y tế Đồng Nai bao gồm cả số liệu của bên B đã được Bảo hiểm y tế thanh toán.
Có trách nhiệm chuyển cho bên B số tiền bảo hiểm y tế còn lại mà bên B đã được Bảo hiểm y tế Đồng Nai thanh toán thông qua tài khoản của bên A.

Điều 2 : Điều khoản chung
Hai bên cam kết thực hiện tốt các điều khoản trên hợp đồng đã được ký kết.
Trong quá trình thực hiện hợp đồng nếu có vấn đề vướng mắc phát sinh ngoài điều khoản ghi trên hợp đồng, hai bên cùng nhau thương lượng, trao đổi thống nhất trên tinh thần hợp tác để đi đến thực hiện tốt hợp đồng.
Hợp đồng có hiệu lực kể từ ngày 01/04/2015 cho đến ngày 31/12/2015.
Hợp đồng này được lập thành 4 bản, mỗi bên giữ 02 bản có giá trị như nhau.
