3.16. Hoạt động theo hợp đồng

3.16.1. Bất kỳ hoạt động nào liên quan đến việc phân phối thuốc được uỷ quyền cho cá nhân hay tổ chức khác thực hiện đều phải được tiến hành dưới dạng một hợp đồng bằng văn bản, được thống nhất giữa bên hợp đồng và bên nhận hợp đồng.

3.16.2. Bản hợp đồng phải nêu rõ trách nhiệm của mỗi bên, trong đó có yêu cầu về tuân thủ nguyên tắc “Thực hành tốt phân phối thuốc”.

3.16.3. Tất cả các bên nhận hợp đồng đều phải tuân thủ các quy định của hướng dẫn này.

3.16.4. Có thể chấp nhận hợp đồng phụ với những điều kiện nhất định trên cơ sở sự chấp thuận bằng văn bản của bên hợp đồng, đặc biệt đối với các hoạt động như lấy mẫu, phân tích, đóng gói lại và dán nhãn lại.

3.16.5. Tất cả các bên nhận hợp đồng đều phải được thanh tra, kiểm tra theo định kỳ.

3.17. Tự kiểm tra

3.17.3. Tất cả những lần tự kiểm tra đều phải được lưu hồ sơ đầy đủ. Trong báo cáo phải có đầy đủ những ghi nhận trong quá trình thanh tra và đề xuất những biện pháp khắc phục, nếu phải thực hiện. Phải có chương trình sửa chữa những sai sót được phát hiện trong quá trình kiểm tra. Những biện pháp đã thực hiện cũng phải được ghi vào hồ sơ. Bộ phận quản lý phải đánh giá báo cáo kiểm tra và những hành động sửa chữa và phải ghi vào hồ sơ.
