Cả hai bên cùng cam kết:
- Những thông tin đã ghi trong hợp đồng này là đúng sự thật;
- Thực hiện đúng và đầy đủ các thỏa thuận đã ghi trong hợp đồng này;
- Việc hai bên lập và ký kết hợp đồng này là hoàn toàn tự nguyện, trong lúc tinh thần minh mẫn và sáng suốt, có khả năng nhận thức và tự chịu trách nhiệm về hành vi của mình;
- Đã được công chứng viên giải thích, đã hiểu rõ và cam kết thực hiện nghiêm chỉnh các quyền và nghĩa vụ của mình theo các thỏa thuận trong hợp đồng này cũng như các qui định của pháp luật về việc ủy quyền.

ĐIỀU 5: ĐIỀU KHOẢN THI HÀNH
- Hai bên cam kết thực hiện nghiêm chỉnh các điều khoản đã thỏa thuận trong hợp đồng. Khi có tranh chấp hai bên gặp nhau cùng thương lượng, nếu không giải quyết được bằng thương lượng thì yêu cầu Nhà nước có thẩm quyền hoặc yêu cầu Tòa án giải quyết;
- Mọi sửa đổi, bổ sung của hợp đồng này chỉ có giá trị pháp lý khi được hai bên thỏa thuận bằng văn bản và được cơ quan có thẩm quyền chứng nhận;
- Những nội dung chưa được hai bên thỏa thuận trong hợp đồng này sẽ được thực hiện theo qui định của pháp luật;
- Hợp đồng ủy quyền này không có thù lao.

Hợp đồng này được lập thành 03 (ba) bản chính có giá trị pháp lý như nhau:
- Mỗi bên giữ một bản;
- Một bản lưu tại nơi chứng nhận Hợp đồng này.

Sau khi đọc lại, hai bên khẳng định đã hiểu rõ, hoàn toàn nhất trí với các điều khoản đã thỏa thuận trong hợp đồng này và cùng tự nguyện ký tên dưới đây.
