A. Thời gian xảy ra tai nạn lao động: 11h07 ngày 22/09/2015.
B. Tên đơn vị và tên nhân viên: Bộ phận Ruột bóng, Nguyễn Ngọc Hoàng (MST: 5272)
C. Tổng chi phí công ty chi trả: Tạm ứng 8.000.000 đồng
D. Tổng số ngày nghỉ: 52 ngày (đến hiện tại)
E. Quá trình xảy ra tai nạn lao động: Trong lúc thực hiện thao tác tại máy hấp ruột banh, công nhân Nguyễn Ngọc Hoàng quan sát thấy ruột banh hấp trong khuôn bị lệch đã dùng tay (T) chỉnh lại trong khi đã nhấn nút hạ khuôn dẫn đến khuôn hấp dập trúng tay (T).
F. Nguyên nhân xảy ra tai nạn: Công nhân không thực hiện đúng thao tác kỹ thuật an toàn khi vận hành máy hấp ruột.
G. Ghi chép xử lý tai nạn:
Ngày 22/09/2015 khi thực hiện thao tác tại máy hấp ruột banh, công nhân Nguyễn Ngọc Hoàng bị khuôn dập trúng tay. Anh Hoàng được chuyển lên bệnh viện Bà Rịa để điều trị. 3h cùng ngày, Anh được chuyển lên bệnh viện Chấn Thương Chỉnh Hình - TP HCM tiếp tục điều trị. Buổi tối cùng ngày, Anh được chuyển sang bệnh viện Chợ Rẫy - TP HCM tiếp tục điều trị. Anh Hoàng được bác sĩ kết luận bị dập và bỏng 5 ngón tay
Ngày 1/10/2015 – 22/10/2015 tiến hành: Phẫu thuật lần I,II,III,IV ( Căn cứ theo Giấy chứng nhận thương tích – số 1318/YC-BVCR)
Ngày 22/10/2015 Anh Hoàng được xuất viện.
Ngày 22/10/2015 – 16/2/2016 Anh Hoàng được theo dõi hồi phục, tái khám 1 tuần 1 lần.
Ngày 17/2/2016 tiến hành phẫu thuật lần V : Cắt cụt đầu xương ngón tay.
Ngày 19/2/2016 Ban chấp hành công đoàn cùng bộ phận ESH dự kiến thăm hỏi động viên Công nhân Hoàng.
